The Destiny Church leader was travelling on Hamurana Road north of Rotorua when the car he was driving collided with another carrying two elderly passengers.
The crash happened on 6 May, and one of the elderly passengers had to be taken to hospital.
The police summary of facts was published today by the Rotorua Daily Post Weekend.
The summary alleges Tamaki was driving east on Hamurana Road on 6 May at around 5pm when it crossed the centreline and crashed into the westbound car containing the elderly couple. The elderly man tried to swerve right to avoid the collision but both passenger sides of the vehicles crashed into each other at speed.
Tamaki has been charged with careless driving and must appear in the Rotorua District Court on 11 August to enter a plea.
