A man in his 50s, identified as Mr A, died of sepsis at Auckland City Hospital following multiple failures and delays in treatment after a stem cell transplant for blood cancer.
The Health and Disability Commissioner’s report revealed that despite his initially expected recovery, his deterioration was not promptly recognised, leading to missed opportunities for life-saving intervention.
His widow described the experience as frightening, as they were informed of his condition without support.
Deputy Commissioner Vanessa Caldwell criticised the lapses in care and directed Health NZ to formally apologise and implement better sepsis management protocols.
Health NZ acknowledged the failings and pledged improvements to prevent similar tragedies.

We lost our 73 y/o Grandmother due to septicemia.
She displayed all of the classical symptoms as seen on a brochure in the lobby of the Chch Hospital.
She was admitted to Chch Hospital (CDHB) and placed in an overflow ward, with severe itching, and was hallucinating.
She then went into a coma…
She went into intensive care after the coma appeared, and only then did any antibiotics get administered via an IV drip.
In addition, she only had one kidney, and they over-loaded her urinary system, stating the the IC ‘team’ were trying to ‘jump-start’ ker remaining kidney.
Then they said that her organs were failing!
She died the next day, but the vultures wanted to do ‘studies’, which we refused.
