
A man in his 50s, identified as Mr A, died of sepsis at Auckland City Hospital following multiple failures and delays in treatment after a stem cell transplant for blood cancer.
The Health and Disability Commissioner’s report revealed that despite his initially expected recovery, his deterioration was not promptly recognised, leading to missed opportunities for life-saving intervention.
His widow described the experience as frightening, as they were informed of his condition without support.
Deputy Commissioner Vanessa Caldwell criticised the lapses in care and directed Health NZ to formally apologise and implement better sepsis management protocols.
Health NZ acknowledged the failings and pledged improvements to prevent similar tragedies.
