Pharmacy error leaves woman overdosed on fentanyl patch

A Dunedin woman suffered a serious overdose after being dispensed fentanyl patches six times stronger than prescribed.

Within hours of applying the patch, she became violently ill and later discovered she had been given the wrong dosage.

The pharmacy apologised, offered compensation, and attributed the incident to human error, while the Pharmacy Council confirmed it is investigating.

The Ministry of Health and Medsafe are also involved, following recent revelations of another fatal pharmacy dispensing error.

The woman says she is still recovering weeks later and laid a complaint to ensure no one else suffers the same ordeal.
