An internal investigation has confirmed that a heavily pregnant woman in her 20s and her stillborn child died due to delayed admission to Palmerston North Hospital’s intensive care unit (ICU) in February 2022.
The woman, whose identity is protected by a coroner’s order, showed signs of severe sepsis upon her early morning arrival but was not admitted to the ICU until the evening. Despite the clear indication of her critical condition, the delay proved fatal, and she passed away later that night.
Health New Zealand eventually released a summary of its internal report to state media today highlighting multiple failures in diagnosis, treatment, and documentation.
The report identifies significant delays in diagnosing sepsis, identifying the infection source, and initiating treatment. There were also numerous gaps in documentation, communication, cultural responsiveness, and fetal monitoring.
Recommendations include revising escalation pathways for deteriorating patients, refresher training for structured communication, proper documentation of maternal observations, and familiarization with fetal surveillance guidelines. Additionally, the report suggests reviewing perimortal caesarean procedures and ensuring prompt availability of surgical equipment.
However, state media’s further inquiries into the implementation of these recommendations received limited responses, citing the ongoing coroner’s case.
The incident has also exposed underlying issues at Palmerston North Hospital, including concerns about staff intimidation and bullying within the ICU. A post-incident report indicated a “siege mentality” among doctors, who feared raising concerns and faced obstacles in patient transfers between departments. Despite planned refurbishments and staffing improvements for the ICU, progress has been slow, with no definitive timeline for completing the new acute services block. This case is now under investigation by both the Health and Disability Commissioner and Coroner Meenal Duggal.
